Sorry, I am not sure I understand the question. If you want to permanently delete a slave, then you would launch the monitor, and right click the slave, choose delete slave, and then uninstall the software on the machine. That would both remove the entry in the monitor and prevent the slave from launching again. Hope that helps.

So, to see if I understand, you have formatted a machine completely, but when it comes back up, it still has the old slave instance, plus the new one? Or is it the assigned username when connecting for the first time that is still there? If the machine is launching 2 slave instances, you should be able to close one and delete it from the machine via the launcher. If it is a username issue, you can just right click the launcher and choose change user.
